Abstract :
Background: Chronic unilateral chest wall pain is a complex and difficult to treat condition. We present a case that demonstrate the safe and effective use of pulsed radiofrequency ablation to achieve significant pain relief.
Case and Diagnosis: A 37-year-old male with past history of stroke 6 months back on antiplatelet since then. Currently complaining of unbearable chest wall pain on left side which has worsened over 6 months. Cardiac stroke was ruled out and related investigations were normal. Patient only had mild pain relief with oral and iv analgesics.
MRI Thorax suggested Posterior disc bulge at D6-D7 level causing indentation of anterior surface of cord and severe right neural foraminal narrowing.
Intervention: Fluoroscopy guided D6-7 Thoracic pulsed radiofrequency ablation was performed after stimulating and confirming the area of pain at D6-7 level. Procedure was uneventful.
Outcome: Patient had immediate relief from pain postoperatively. one month after procedure patient was off oral analgesics. VAS score was 1 point
Conclusion: Our case suggests that thoracic pulsed radiofrequency ablation can be used safely to treat patients suffering from chronic chest wall pain.
