Abstract :
Introduction:
Dorsal root ganglion stimulation is an emerging therapy in treatment of chronic pain. Compared with traditional spinal cord stimulation, it allows discretely targeted stimulation profile.
Methods:
During 2020-2022, we have performed 21 dorsal root ganglion stimulator implantations in Cleveland Clinic, Abu Dhabi. The indications were post herpetic neuralgia, diabetic neuropathy, and groin pain. All patients underwent trial before final implantation.
Outcome was measured using PHQ-9, PDI, EQ-5D, and GAD 7.
Results:
85% of the patients experience significant improvement. The improvement was persistent with time. Lead migration was noted for one patient. The leads are still not MRI compatible and two patients underwent explant when MRI was needed for different reasons.
Explant procedure can be more difficult technically in compare to explantation of spinal cord stimulators.
Conclusion:
Targeting DRG has several advantages, including lower energy usage, more focused and posture-independent stimulation, and reduced paresthesia. However cases have to be selected carefully to ensure improved outcome.
